Parent / Guardian Consent

As the parent or legal guardian, you consent to your child participating in G-Code Performance youth training, including boxing-based development, conditioning, and coordination work.

You authorize G-Code Performance to seek emergency medical care for your child if you cannot be reached, and confirm the emergency contact details you provided are current.

You are responsible for drop-off and pick-up at the scheduled session times.
